Job Description

Under the supervision of the Vice President of Operations, the Program Director develops the program content in accordance with the needs of participants, their families, California’s Bridge to Reform Demonstration 1115 Waiver Community-Based Adult Services (CBAS) requirements and Title 22 Adult Day Health Care (ADHC) regulations for Choice in Aging’s Adult Day Health Care Centers. The Program Director is at the mentor level and is expected to promote the agency in all interactions. In addition, the Program Director works as a member of the Choice in Aging’s team to meet our mission to provide the services needed for frail, elderly and disabled adults to remain independent and retain their dignity while engaging in an active life in the setting of their choice.

Essential Functions:

  • Develop and maintain the program in accordance with Title 22 and the needs of the participants served.
  • Responsible for scheduling and coordinating the ADHC’s person centered care programs and employees.
  • Conduct ongoing evaluation of the program and formulate adjustments to the program to meet participant needs and program regulations.
  • Coordinate all team members of the program in the delivery of services. Meet daily with the ADHC staff to discuss current issues.
  • Prepare for and attend monthly management meetings led by the President & CEO.
  • Update and maintain policies and procedures as they pertain to our ADHC programs. Be familiar with the disaster plan, review annually and ensure all employees know their specific responsibilities.
  • Responsible for accurate documentation of staff and consultants hours and complete bi-weekly payroll approvals in an accurate and timely manner.
  • Hire, train, coach, supervise and complete performance evaluations of those employees and volunteers under her/his supervision, including the Interdisciplinary Team (IDT).
  • All other duties assigned by supervisor.

Minimum Qualifications:

  • A Program Director is a person with the following:
    • One of the following backgrounds:
      • A person with a bachelor’s degree in the field of nursing, social work, psychology or recreational, occupational or physical therapy with demonstrated or potential competence in working with the impaired, elderly living at home and a minimum of two years of experience in a management, supervisory, or administrative position
      • A person with a master’s degree in the field of nursing, social work, psychology or recreational, occupational or physical therapy with demonstrated or potential competence in working with the impaired, elderly living at home and a minimum of one year of experience in a management, supervisory, or administrative position
  • Person will be of flawless integrity with the appropriate skills, knowledge, and abilities related to the health and mental, cognitive, and social needs of the participant group being served by the adult day health care center, including the elderly and adults with special needs;
  • Person shall be knowledgeable of physical, social and mental health programs operating within a licensed health facility or clinic. CBAS and/or ADHC experience is highly preferred.

Choice in Aging (CiA) has been providing services to the community’s frailest people since 1949, when it was created by volunteers to serve children severely disabled by Polio. Over the years, the organization has provided a wide variety of programs, with the goal of promoting dignity and independence of people with disabilities and special needs. Our mission is to create opportunities where people can learn, grow, and age independently with dignity in community. Program History Adult Day Health Care (ADHC) is a program that was created in California in 1978 in response to a need to have an alternate solution to institutionalization. The program mirrors the skilled nursing staffing model, but is based in the community and typically operates during the day on weekdays. It is akin to a senior center for those who can no longer attend a senior center independently. It is especially beneficial for those who need additional support to do things like ambulate, eat, toilet, participate in activities and more. Staff provides therapeutic activities disguised as fun throughout the day, making socializing and exercising meaningful and pleasurable. This makes folks want to come back each day!
